Makeup Test Request Form for Instructors

Please observe the following guidelines for allowing makeup tests for a student who may not be able to take a test/exam at the scheduled class time because of an illness or other extenuating circumstances. Under these circumstances, an instructor may submit a request for the Placement and Testing Services to proctor the test for the student.

Guidelines

A student may not be able to take a test/exam at the scheduled class time because of an illness or other extenuating circumstances. Under these circumstances, an instructor may submit a request for the Placement and Testing Services to proctor the test for the student. To make a request, please follow the process outlined below:

  1. Complete and submit this Request Form to the Placement and Testing Services (Test Center).
  2. Instructor is responsible for arranging a suitable time for the student before submitting the request.
  3. Request must be submitted no less than 24 business hours before requested test time.
  4. The Associate Director or other testing personnel will review the request and respond by email as soon as possible.
  5. If approved, Placement & Testing Services will be responsible for administration and collection of exam.

Makeup test proctoring is available during the timeframes listed below. The test time limit must fit entirely within these hours. Please note: We continue to offer makeup exam service through the breaks between quarters, including August-September.

  • Mondays from 9 a.m. to 12 p.m.
  • Wednesdays from 1 p.m. to 4 p.m.
  • Thursdays from 11 a.m. to 2 p.m.

Spring Quarter (2025) Added Days

  • Friday May 23: 9:00 a.m. – 12:00 p.m. (added day due to holiday May 26)
  • Tuesday May 27: 9:00 a.m. – 12:00 p.m. (added day due to holiday May 26)
  • Tuesday June 17: 9:00 a.m. – 1:00 p.m. (added day due to holiday and coming finals)
  • Wednesday June 18: 12:30 – 4:30 pm (extended hours due to holiday and coming finals)
  • Friday June 20: 9:00 a.m. – 12:00 p.m. (added day due to holiday June. 19)

Spring Quarter (2025) Extended hours for FINALS WEEK

  • Monday June 23: from 9 a.m. to 1:00 p.m. extended hours this day
  • Tuesday June 24 from 9:00 a.m. to 2:00 p.m. (added day due to FINALS)
  • Wednesday June 25: 1:00 – 4:00 p.m. usual hours offered
  • Thursday June 26: from 11 a.m. to 3 p.m. extended hours this day
  • Friday June 27: 9:00 a.m. – 12:00 p.m. (added day due to FINALS)

CLOSED DATES (SPRING ’25 QUARTER)

  • Please note: a request submitted on the below dates will be processed 24 business hours later
  • Tuesday April 29 – Campus Community Day
  • Monday May 26 – Holiday
  • Thursday June 19 – Holiday

Please Note

Placement and Testing Center cannot be used to administer multiple versions of an exam to a student or group of students, and the Test Center does not have the capacity to administer an exam to an entire class. (This last restriction does not apply to online classes.)
