Application Steps
Step 1: Review the Application Checklist
Before you begin the application process, review our application checklist [PDF]. It's your responsibility to make sure that your application is complete and submitted before the deadline for admissions consideration.
Step 2: Apply for College Admissions
If you are already a Bellevue College student (within the last 4 academic quarters), you should skip to Step 3.
If you are new to Bellevue or have not attended in the last 4 academic quarters, you must apply for college admissions in addition to applying to our program.

- Or include a printed college admissions application [PDF] along with your Diagnostic Ultrasound Technology program application packet.
Note: to avoid paying an additional fee for the college application, you should apply for the college through the Apply Online link on this website, or include your printed application when you submit your Diagnostic Ultrasound Technology program application.
Step 3: Complete the Diagnostic Ultrasound Technology Application
To Apply Online
- Complete a Diagnostic Ultrasound Technology Program Application online.
- Pay the $40.00, non-refundable application fee online. Note: Applications will not be processed until this fee has been paid.
- Create a Personal Video Statement [PDF Instructions]
- Mail or hand-deliver your:
- sealed, official transcripts
- personal statement
- a resume, citing education and work experience
- 2 copies of your Personal Video Statement
To Apply by Mail
- Complete a Diagnostic Ultrasound Technology Program Application [PDF]. Note: handwritten forms will not be accepted!
- Include the $40.00, non-refundable application fee.
Check or money order payable to: Bellevue College. We cannot accept cash, debit or credit payments for paper applications. - Create a Personal Video Statement [PDF Instructions]
- Include your:
- sealed, official transcripts
- your personal statement
- a resume, citing education and work experience
- 2 copies of your Personal Video Statement
Step 4: Submit Application Materials
Mail or hand-deliver your transcripts, personal statement and/or complete application (including payment) to:

Applying for Additional Programs
Some students apply for more than one program within Radiation & Imaging Sciences. For instance, you might want to apply for both the Radiation Therapy and Radiologic Technology programs. If you would like to do this, please note that you will need to complete an application and pay an application fee for each program.
You should also know that each program has unique requirements, procedures, and application periods/deadlines. It’s your responsibility make sure that your application is complete and submitted before each deadline.
