Application Documents
Your application should include:
1. Completed program application
- Type or print legibly!
- Print a copy of the application [PDF]
- Read our Program Application Checklist
2. Completed Bellevue College admission application
If you are not a Bellevue College student, complete an application for admission to the college, but do not complete this application online!
Do NOT complete a Bellevue College application online; if you do, you will be charged the college's application fee in addition to the $40.00 Imaging Sciences application fee!
3. A completed DUTEC application worksheet
- Please type or print legibly!
- Download and Print a DUTEC Application Worksheet [PDF]
4. Sealed copies of your transcripts
- One (1) official copy from each college you have attended for prerequisite courses.
5. $40.00 application fee
- Check or money order payable to Bellevue College for non-refundable program application fee. We will not accept cash or credit card information.
6. Cover letter
- An application cover letter describing your work history and health care related experience.
7. Resume
- A copy of your current resume including one reference person with contact information. No reference or recommendation letters are needed.
8. Two (2) video statement DVDs
- Provide two copies of your video statement DVDs.
