Required Documents for Admissions and Enrollment
To ensure your health and safety and that of the of the clients in health care agencies that provide clinical learning experiences, the following documentation is required.
Note: Lack of compliance with any of these requirements will prevent a you from entering the program.
Records and Documents Required for Admissions
These documents must be submitted at the time you complete your application.
Note - Beginning August 2012, Background Check Report is not required when you apply. You will be asked to run/pay for it if you are admitted to the program.
- Conviction/Criminal History Disclosure Form
Download the PDF file from the Application Instruction page (Step 2) and complete the form.
- CPR for Healthcare Providers (American Heart Association)
A current copy of CPR card must be submitted.
The CPR card must be issued by a person or facility qualified specifically to instruct CPR for Healthcare Providers (American Heart Association) that includes infant, child and adult. Other types of CPR certification will not be accepted.

- Immunization Records
You must submit the following immunization records. Your name must be on each piece of documentation.
Immunization/Test Details Tuberculosis (TB) Test
You must submit ONE of the following documentation:
Negative Tuberculin skin test (Two step PPD test) results:
A copy of two-step PPD test results within the past 12 months (Two injections & two readings given 1-3 weeks apart). The document must include the size of induration in millimeter (mm) and negative result.
If you had a positive PPD test, you must provide the documentation of treatment/Chest X-Ray and current clearance from physician. This letter must be dated within the past 6 months.
Negative Annual PPD test results:
A copy of your current annual PPD test and a previous year's PPD test. If your annual PPD test lapses and it is more than 12 months apart (i.e. 365 days), you will need to complete the two step PPD test process. The document must include the size of induration in millimeter (mm) and negative result.
Negative QuantiFERON TB blood test result
A copy of QuantiFERON TB Blood test within the past 12 months.Hepatitis B (HepB) You must submit BOTH documentation:
Complete 3-dose series of Hepatitis B vaccines*:
A copy of three HepB vaccines. Please note that it takes 5-6 months to complete all 3 series
Positive HepB (HBs) titer result:
A copy of Hepatitis B Antibody titer (blood test) resultMeasles, Mumps, Rubella (MMR) You must submit ONE of the following documentation:
Two doses of MMR vaccines:
A copy of two MMR vaccines
Positive MMR titer result:
A copy of Measles, Mumps and Rubella titer resultsTetanus, Diphtheria, Pertussis (TDAP) A copy of TDAP vaccine/booster within the past 10 years Varicella (Chicken Pox)
You must submit ONE of the following documentation:
Two doses of Varicella vaccine:
A copy of two Varicella vaccines
Positive Varicella titer result:
A copy of Varicella titer results
Influenza You must submit ONE of the following documentation:
A proof of annual influenza vaccine:
A signed waiver for students who decline the vaccine:*
* Specific healthcare institutions may require vaccine for all students or may ask students to obtain a medical waiver from physician.
Documents and Records Required for Enrollment
Once you have been accepted into the Nursing Program, you must submit the following documents by the specific due date indicated by the program.

| AIDS/HIV Training Certificate | A copy of AIDS/HIV 7 hour course Certificate of Completion. |

Nursing Assistant Certification |
Before starting the program (i.e. First day of the class), you must obtain a Certificate of Completion (NAC) from a two-year community or technical college. If you attended a non-community/technical college program, you must obtain a Washington State NAC license. * CNA Program is offered at Bellevue College.
