Information for Admitted Students

To ensure your health and safety and that of the of the clients in health care agencies that provide clinical learning experiences, the following documentation is required prior to enrollment in the nursing program. 

Note: Lack of compliance with any of these requirements will prevent a student from entering the program.

Immunization Records

You must submit copy of immunization records signed by your healthcare provider.  Your name must be on each piece of documentation.

Immunization/Test Details
Negative tuberculin skin tests (PPD) or chest x ray (2 step PPD) A copy of the two-step PPD (two tests given three weeks apart), or release documentation from Public Health or own primary care provider.  The test must be kept current (within the past 12 months).

Persons with positive PPD must provide documentation of treatment and current clearance (freedom from symptoms) from Public Health or own primary care provider.  This must be dated within the previous six months.
Hepatitis B vaccinations (complete series of 3)

or


positive Anti Hep B titer
You must have 3 vaccination series




or
positive Anti Hep B titer
Measles, Mumps, Rubella (MMR) immunization (2) ,

or


positive titer
Two Measles/Mumps/Rubella vaccines or providers documentation of rubella and rubeola diseases



or
positive titer results
Tetanus/Diphtheria (Td) Tetanus/Diphtheria (Td) immunization within last 8 years is required.
Varicella (Chicken Pox) (2)

or


Positive Varicella Titer
Two doses of varicella vaccine (4-8 weeks apart)


or Positive varicella titer, or Birth in U.S. prior to 1965, or prior to 1997 with history

Required Documents

Additionally, you must submit the following documents:

Document Details
CPR CARD Current CPR card for Healthcare Providers

Note: Other types of CPR certification will not be accepted.

The CPR card must be issued by a person or facility qualified specifically to instruct CPR for Healthcare Providers (e.g. the American Heart Association) that includes infant, child and adult.
Proof of Military Duty Registration All male students between 18 and 25 years of age must show proof of military duty registration.  For information visit http://www.sss.gov/when.htm.
AIDS/HIV Training Certificate A copy of AIDS/HIV 7 hour course Certificate of Completion. 
Nursing Assistant Certification Before starting the nursing program, a Certified Nursing Assistant (CNA) certificate must be obtained and complete before the first day of the quarter.

Note: Obtaining the Washington State CNA license is no longer required, but  strongly recommended.
Criminal  Background Check A criminal history background check is required by clinical agencies.  You should complete the background check online at http://www.certifiedbackground.comand submit a copy of your result summary. 

A national check going back seven years is required.
